Primary Functions:

Responsible for the health and safety of the clients and the operation of his/her shift.

  • Carry out the philosophy, goals and objectives of New Seasons, Inc.
  • Implement M.D. Orders
  • Assessment, Planning, Implementation & Evaluation of Nursing Care
  • Developing Nursing Care Plans
  • Coordination of hospital and discharge planning for individuals being re-admitted to New Seasons facilities following hospitalization in order for the individuals needs to be met, including but not limited to DDS and Visiting Nurse services as needed.
  • Collaborate with Allied Health Professionals
  • Perform monthly reviews of kardexes and physicians’ orders for each individual.
  • Order client medications per doctor’s orders.
  • Administer medications.
  • Assist with daily client care.
  • Monitor client’s weights for loss/gain.
  • Check to see that special diets/consistencies are being followed, food preparation, feed clients.
  • Tracking medical appointments to ensure that all medical/dental needs of the individuals are being met.
  • Attend individual medical appointments
  • Perform ADL needs.
  • Monitoring for OSHA compliance.
  • Purge medical records.
  • Keep RN Supervisor informed of client conditions.
  • Attend In-Service training and staff meetings as required.
  • Developing Nursing Care Plans
  • Documentation, write nursing reviews as needed – weekly, monthly, quarterly.
  • Counseling/disciplining staff when indicated for problems involving medical/medication issues.
